Practice Patient Portal
The practice has a new Patient Portal for secure remote access to your health record.
For new and established patients it is as easy as calling the office to give us your email address and we email you your log in credentials (615) 865-7050. The portal will guide you from there. You can enter your health information ahead of your appointments to save you time while you can leisurely enter in complete information without feeling rushed in a waiting room.
